This public website provides general information about Weber Behavioral Health. It is not a secure clinical communication channel. Do not use the website, ordinary email, or social media to send medical information, urgent requests, insurance identifiers, or information needed for treatment.

This website does not use advertising trackers or visitor-level analytics. Standard hosting security logs may temporarily record technical information such as an IP address, browser type, requested page, and timestamp.

Public contact form

The contact form is intended only for general scheduling and administrative requests. Form submissions are processed by Web3Forms. Do not include protected health information or sensitive medical details. If you prefer not to use the form, call the office at (402) 759-3802.

Appointments and clinical records

Use an approved scheduling or secure intake service for clinical information. Clinical records submitted through a future secure client portal will be governed by the practice’s Notice of Privacy Practices, not this public website policy.
